CPA LICENSE LATE RENEWAL APPLICATIONS (PDF)

Late filing fees are applicable to all licensees whose Ohio permit or Ohio registration expired on December 31, 2008. See the late filing fee chart for the correct fee.

NOTE: Additional fees would apply if a licensee's Ohio permit or Ohio registration expired December 31, 2007 or earlier.

All licensees who renew the Ohio permit after the renewal deadline are required to submit documentation of 120 CPE credits, earned during the reporting period January 1, 2006 to December 31, 2008. Examples of appropriate documentation include certificates of completion, grade reports or transcripts, and other similar evidence. Licensees must submit documentation for all CPE programs listed on the CPE Report Form.
