This form is designed for continuing education sponsors who wish to renew their sponsor status with the Board office. It should not be used for initial sponsor registration. Initial CPE sponsors should use the initial CPE sponsor registration form (Adobe PDF format) which may be downloaded and sent to the Board along with the appropriate supporting documentation. Verification of sponsor registration will be mailed either electronically to the e-mail address designated on the renewal form or sent by regular mail, and the Ohio sponsor list on our website will be updated. Remember that all CPE sponsors agree to follow the NASBA CPE Standards as a condition for sponsor renewal with the Board.
