Requests for name changes must be made in writing (email is acceptable) and include an official document that reflects the name change (marriage license, divorce decree, etc.), plus the licensee's certificate/registration number (CPA.xxxxx, PA.xxxxx) and either the licensee's driver license number or the last 4 digits of the licensee's social security number.
